The radiator fan and the air conditioner condenser fan rotate at low speed, but the radiator fan does not rotate at high speed when the coolant temperature is above 96.5°C.

Check for an open in the wire between under-hood fuse/relay box connector #3 terminal #4 and ECM connector terminal A5.
